If you contact us or request an appointment, you may provide your name, email, phone number, requested service, preferred date and time, and notes. The appointment request form creates a draft in your email application; you choose whether to send it. We use information you send to respond, discuss availability, arrange services, and manage our relationship with you. Career inquiries may include qualifications and work experience.
Please keep website inquiries to scheduling and general questions. Do not include medical records, detailed health information, or payment card details in the request form or ordinary email. Ask the relevant practitioner how to provide information needed for your care.
Square appointment booking and payments
Our booking links open external Square-hosted pages at square.site or book.squareup.com. Square Appointments handles that booking experience. Depending on the service and booking settings, Square may request contact details, appointment selections, notes, and payment information. Square provides booking information to the relevant service provider to arrange and manage your appointment. Payments or cards on file, when requested, are handled through Square rather than this website's request form.
Square processes information under its own notices and may use cookies, device information, and other technologies on its booking pages. Review Square's privacy notice for customers and, when relevant, its Buyer Features privacy notice. Our cookie preference controls apply to this website and do not change Square's settings. Review any booking, cancellation, or payment terms shown by the provider before confirming.
Website analytics and technical information
With your acceptance of optional analytics, we use Google Analytics 4 (measurement ID G-HG2M8WVCM3) to understand website use, including pages viewed and device, browser, and approximate geographic information. Google processes network information, including IP addresses, to deliver the service. Our tag removes query strings and fragments from the page URL it supplies to Google, and does not enable Google signals or advertising personalization. Analytics can reveal which service pages you visit. We do not intentionally send appointment form contents to analytics.

Our hosting and email providers may process technical or communication data to deliver and protect their services. The website also loads a font from Google Fonts, which sends network information such as your IP address and browser headers to Google when the font is requested. This is separate from optional Google Analytics.
Other providers and sharing
We use service providers for website hosting, email, analytics when accepted, and booking. Information may also be disclosed when required by law or needed to protect safety or legal rights. Links to associates and Anywhere Clinic take you to independent providers; their privacy practices and any clinical records are governed by their own notices. This website policy does not replace a practitioner's patient privacy notice.
Retention, security, and your choices
Information may be retained as needed to handle your request, provide services, maintain business records, and meet applicable obligations. Retention within Square, Google, and other providers also depends on their policies and account settings. No internet transmission or storage method is completely secure.

For information controlled by Square or another independent provider, contact that provider as well.
